Dear Parents/Guardian:

Attached are the medical forms required by the State of New Jersey for attendance at the Academy of St. Elizabeth. Beginning in the 2007/2008 school year we strictly require a complete annual physical examination, a health history Questionnaire, and immunization records. Only physicals on this form will be accepted. Immunizations must be complete and up to date and each one recorded with the month, date and year. If your daughter intends to try out for an athletic team at the Academy these forms are due by August 1. All other students' forms are due by September 1.

Annual physicals are good for exactly one year from the date of the physical. Please plan your doctor's appointment for a physical accordingly. It is recommended that the physical be scheduled during the summer months to ensure eligibility for the entire upcoming school year. The health history questionnaire filled out by the parents or guardian is required for students who wish to participate in athletics and whose physical is more than 60 days old. This will conked mainly winter and spring teams, as well as fall participants who had a physical early in the summer.

New Jersey State rules and regulations require that ALL MEDICINE (prescription and over the counter) be accompanied by written permission from BOTH the PARENT and PHYSICIAN. Many students come to me requesting medication for cramps, headaches, etc, that I can not give because I do not have the required authorization. I have included separate forms for over the counter medication administration, prescription medication administration and for self administration of inhalers and epi pens.

All Over the counter Medication shall be brought to school in the original sealed container and labeled with the student's name. In the past I have kept Advil, Tylenol and cough drops for students who had permission but no medication. I will no longer be keeping a supply of any medication. Prescription medication shall be brought to school in the original prescription container, labeled with the name of the student, medication, dosage and name of the physician.

For freshman and new students: Attached is a form for you to give to your daughter's current school nurse requesting transfer of her medical records to the Health Department at the Academy of St Elizabeth.
